Medical Check-Up for Visa Applicants for Australia, New Zealand, and Belgium

We provide  medical check-ups for visa applicants to Australia, New Zealand, and Belgium.

Please use the correct check-up application form for each country.

The check-up fees and procedures are different depending on the country which you apply for. Please refer to the following details.

  1. Please apply at least one week before your requested test day. 
  2. An application made after 15:00 on Friday will be reviewed and replied to next week.

<How to Get Tested>

 

1. Book online
  • Please apply using the application form below.
  • We will contact you after we have received your application.
  • You will be requested to email us a copy of your passport identification page in advance of your appointment date.

 

2. Visit on Your Appointment Day
  • Please come to the Medical Checkup Center on the 3rd floor. If you will use our interpreter services, please first stop at the general reception on the 1st floor.
  • (Only for Belgian visa applicants) Please bring a Letter Pack envelope with your completed address if you wish to receive your certificate via post. You should be able to receive your certificate one week after the medical check-up.
 
3. After the Medical Check-Up
  • For Belgian visa applicants: Please visit the hospital one week after the medical check-up and pick up the medical certificate. If you requested to receive it via post in advance, we will send it out as soon as it is ready.
  • For Australian & New Zealand visa applicants: We will upload the test results directly to eMedical.

 

For Australia Visa

Pregnant ¥24,200 AU basic items, blood test, urine test
501 Standard ¥24,200 AU basic items, blood test, urine test
Under 15 years old ¥16,500 AU basic items, urine test
Under 5 years old ¥11,000 Doctor’s interview, height, weight, hearing test, and head circumference (if under 2 years old)
502 ¥12,100 Chest X-ray
HIV/HBS/HCV ¥3,300 *Only for those who are applicable. Each item costs ¥3,300.

*AU basic items: doctor’s interview, height, weight, blood pressure, eye test, and hearing test.

 

  • Either HAP ID, TRN numbers or Visa Requested ID is required to book an appointment.
  • Those who will take a blood test should not eat anything on the day of your appointment until you have finished the check-up.
  • Those who have a blood test or urine test should refrain from doing any strenuous exercise or drinking alcoholic beverages on the day before the check-up appointment.
  • In the case that additional tests are required, there will be an additional charge for such cases.
  • For those who are under 15 years old and have kidney disease, we might add a blood test.
  • What to Bring:

(1) original copy of your passport

(2) a document which we can confirm your HAP ID or TRN numbers (e.g., referral letter)

(3) signed client declarations

(4) completed questionnaires

 

 

 

For New Zealand Visa
Fees (tax included, public health insurance not applicable)
Standard ¥30,800 NZ basic items, fundoscopy, chest X-ray, blood test, urine test
Under 15 years old ¥19,800 NZ basic items, chest X-ray, urine test
Under 11 years old ¥16,200 NZ basic items, urine test
Under 5 years old ¥11,000 Doctor’s interview, height, weight, hearing test, and head circumference (if under 2 years old)
Pregnant ¥27,500 NZ basic items, fundoscopy, blood test, urine test
502 ¥12,100 Chest X-ray

  • If you are female and need a urine test, please take the urine test one week after menstruation is finished.

  • Those who will take a blood test should not eat anything on the day of your appointment until you have finished the check-up.
  • Those who have a blood test or urine test should refrain from doing any strenuous exercise or drinking alcoholic beverages on the day before the check-up appointment.
  • In the case that additional tests are required, there will be an additional charge for such cases.
  • What to Bring:

(1) original copy of your passport, (2) completed questionnaires.

 

 

 

For Belgium Visa
Fees (tax included, public health insurance not applicable)
Standard ¥30,800 BE basic items, BMI, ECG, chest X-ray, eye test, hearing test, blood test, and urine test
Under 15 years old ¥11,000 BE basic items
¥19,800 BE basic items, and ECG or chest X-ray
¥22,000 BE basic items, and blood or urine test
Pregnant ¥28,600 BE basic items, BMI, ECG, eye test, hearing test, blood test, and urine test

*BE basic items: doctor’s interview, height, weight, blood pressure, and pulse.

 

  • You will be required to send us a certificate form in advance. Please contact the Embassy of Belgium for further details.
  • If you are female and need a urine test, please take the urine test one week after menstruation is finished.
